I've been told that a Munter is pretty worthless for rescue loads. What would you use for the belay in this situation? I've heard of using a pair of prussiks but that is for lowering situations. It would be a pain in the ass for a raise.

Drop test has shows that Munter can not arrest a single load fall so no one really use it in rope rescue. The two prusik method is pretty standard on belay system however during raising, you keep your hands off the prusik and let a pulley to reset prusik for you.
