I'm leery of posting this thread in this forum since it's meant to be for discussion of specific accidents, but this seems better than the conflation of this discussion with an actual accident report as is currently the case.
As a reminder, the guidelines are:
"This forum is reserved for polite discussion of specific accidents. Please keep posts on subject and respectful of both the people involved and other users."

If we believe that the current guidelines are inadequate, then let's post new ones (ideally with prior discussion) rather than engaging in the somewhat Kafkaesque practice of upbraiding members for violating unpublished guidelines.
I've started a thread here for discussion of revisions to the guidelines.
