Watch in the vimeo video at just before 2:00 he puts his ice tool on his shoulder with the blade near his neck so he can use his hand for something. Um, isn't that kind of dangerous? If he were to have fallen, isn't that a slit throat waiting to happen?

That is likely one of the less dangerous things in the video... and yes, there is an alternative if you feel you can't manage the risk of a "shoulder hook." See Pg. 40-41 of Sean Isaac's Mixed Climbing book for a common alternative, the "Thumb Hook." Shoulder hooks are quite common when mixed climbing, as are thumb hooks. It depends on the person, the moves, and the stance. When mixed climbing, I'd say the shoulder hook is avoided more due to the risk of dropping a tool, as opposed to avoiding an injury. When bouldering with tools, shoulder hooking should be avoided, since you don't want a tool underneath you when you fall!
