to "top out" means to get onto the top of a boulder or cliff.
example:
That top out is super awkward! The way the top is sloping towards you."
or
"Man, I've never solo'd a route as hard as that before, I felt so elated when I topped out".

Most topouts leave you on top of the boulder. Some just have a last jug.

does "pucker-factor" mean irregular fold on the surface of the rock so you need to struggle moving across the difficulties?

I think the first term is self explanatory-- it was funny that it was listed under the letter A
