i found this picture and i noticed several issues... probably none of them are fatal, but i think several of them are at least frowned upon. ps: the instructors call themselves expert climbers/rappelers what do you think??

Interesting you note that there are several issues, but none are fatal. I recently had a discussion with a friend I'm mentoring on "non-fatal errors." We concluded that a lot of accidents happen not because of a single fatal error, but because of two or more "questionable" items. For example, a climber has odd clipping technique, and the belayer short ropes the climber has he clips. This results in the climber to dropping the rope while clipping and taking a large (potential ground) fall. Neither was a fatal error, but a questionable practice (odd clipping technique and/or trying to clip while too pumped) and a minor mistake (short-roping the climber) may have come together to cause an accident.

This is standard way of teaching and securing a beginner to rap and system is perfectly fine

Like SBA said, the pic is way too poor/small to see any of that. At least on my screen. GO
