There was a mistake made with about 15 carabiners made by Metolius. Apparently the safety pin holding the gate in place was not finished. This would allow the pin to slide out. I found this at GearExpress.com.

I think this has already been posted, but as an aside, wouldn't this be a good topic for World Climbing News? No offense at all to Steph Davis, but this is a fairly important subject, all things considered.
