I think the bottom line here is that ATCs are never 100% reliable or not even close. Why don't just use a grigri or any other auto-lock belay devise?

Somehow you've taken away entirely the wrong message from all you've experienced and read. ATCs are 100% reliable. In fact, they're perfect. Grigris are 100% reliable as well. What appears to be unreliable these days is the people using them. The NRAs 70's mantra sums up the situation well. If civil aviation were climbing it would be raining light planes.
