This article in Rock and Ice describes a fall resulting from a 'nose hooked' biner http://www.rockandice.com/...0&type=accidents. I am not sure I understand the term right. Is it just something caught in the gate or a particular configuration that loads it in the wrong spot with the gate closed?

Nose hooking is a serious deal. I tested a hooked biner: Metolius carabiner loaded on the nose (rated 24 kN major axis, 8 kN open, 8 kN minor axis): - Breaking strength: 2.53 kN (failure at spine)
