It might help to explain what the survey information will be used for. Is this research for a business venture? Is this a study for a class? Is it a stalker database? :wink:

Please also include your mother's maiden name, date of birth, social security number, credit card numbers, checking account numbers and routing numbers, ATM PINs, and driver's license number.
