What details there are can be found over on the taco. Short version: She was rappelling off the route from a monitored gear anchor. A cam "popped" and the person watching the anchor replaced it indicating "all go." At some point in the final rappel, the anchor blew resulting in her fall. Observational notes include 1) that the route in question has gear only anchors and that the large blocks that offer horizontal placements are not as bomber as they might appear. A cam or cams could easily lift a car size boulder which could result in the type of failure described. 2) The possibility of setting an anchor farther back edge and extending to the edge was also mentioned as standard procedure. 3) Speculation arose that the controlled environment of he gym giving rise to larger numbers of recreational climbers than ever before in combination with the unpredictable nature of outdoors may be creating an environment in which inexperienced climbers venturing outdoors do not have sufficient mentoring or support to be truly self reliant. May she rest in peace and her friends and family find comfort.
